SIRB Service Letter
A letter to whom it may concern indicating that the University of Utah IRB is willing to serve as the single IRB (SIRB) for a specific study.
Study Support Letter
A letter to whom it may concern to assure the recipient that the IRB plans to review the application for a specific study upon submission to the IRB.
Intent to Rely Letter
A letter to whom it may concern to assure the SIRB or study sponsor that the University of Utah is willing to rely on a defined external IRB for SIRB review of a specific study.

How to Request a Letter

To request a letter of support from the IRB, please submit a Reliance Consultation Request form and clearly indicate what type of letter you need us to provide. Include the email address you’d like the letter sent to, and whether you need a PDF of the letter or if an email from the IRB is sufficient.  If you need the letter urgently or before a specific date/time, please include that information in the request. You may also send urgent requests directly to uuirbreliance@utah.edu.
